If you’ve been off sick for more than 7 days
To request a sick note:
Please complete the Ask GP A Question form explaining that you want a sick note, the duration you think you need it for and the medical reason for the sick note.
To request an extension to a Med3 please complete the Ask GP A Question form explaining that you want a sick note extension, the duration you think you need it for and the medical reason for the sick note.
Your request will be sent to a GP who will either issue the Med3 or if they need further information, reception will get in touch to advise of next steps.
Sick note extensions cannot be requested before the previous one expires so please do not request one before the expiry.
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
